Incoming Students
All incoming
students must complete and return a
Health History Form and
Immunization Record to the
Student Health and Wellness Center.

Forms must be submitted as follows:
1. Via the Web Portal
and
2. A copy signed by your health care provider must be mailed to: West Chester University, Student Health Center, West Chester, PA 19383
If we do not receive your forms by the required deadline , a hold will go on your account. A hold will prevent you from future registration and prevent release of your grades.If you have any questions, call the Health Center at 610-436-2509.
