These documents are mainly for university owned housing.  The following documents can either be read online or downloaded to your computer. To read online please just click on the link, or to download the document to your hard drive please right click on the link and select Save Target As and save the document to your computer.

• Housing Release Request Form (used if you need to request to be released from your university housing assignment during the academic year)
• Residence Hall Occupancy Agreement (this agreement is the “lease” students must sign to live in the Traditional Residence Halls.)

• Information Report (use this form to document interactions with residents or staff (positive or negative), document roommate conflicts, document any information that the RD/GHD/GA should be made aware of.)
• Incident Report  (use this form to document incidents that occur in your housing facility)
• Meningitis Waiver Form (use this form to submit the required meningitis information to the Office of Residence Life and Housing Services)
• Transgender Housing Policy-Housing Accommodation Form (use this form for transgender based housing request)
