Please fill out the following form to complete your course combined request for D2L. You should only request the combination for sites in which you are the assigned faculty.

Please do not start building your course (uploading content, creating quizzes, etc.) until your courses have been combined.

Make sure to list the proper sections for the courses you are combining. Omitting course sections or listing the wrong sections could result in the combination of courses with other Faculty.
